Does the CDC contact you about STDs?

how does the whole process work? i thought doctors only reported to the CDC for statistical purposes, but i was told i was to be contacted about my ual history , etc. That means they have my personal information. do they get your partner's information from you? and then contact them ? If you have any knowledge or experience about this please share. I can't seem to get many good answers on here.
